All Diplomates who passed the exam in 2024 will be emailed a shipping verification form to complete prior to April 25, 2025.
Please double check that you have provided an accurate shipping address on the verification form. If the form is incomplete or inaccurate, we will not be able to send a replacement lapel pin.
