ACLM now offers formal review of health promotion and wellness programs seeking the designation of ACLM Certified Lifestyle Medicine Program. The certification process is designed to support and leverage programs that use lifestyle change as a therapeutic intervention for disease treatment and reversal.
Programs may achieve the status of Honorable Mention, Certified, or Gold Certified depending on their evaluation scores and will be featured on the LMERC website for the benefit of patients, healthcare providers, employers and others who are responsible for their health or for delivering or paying for healthcare on behalf of their employees or beneficiaries.

Process
- Applicants should prepare their application in a separate document (click on the link below to access the application form).
- Submit the application and pay the appropriate fee on the payment processing page. You may select “Mail a check to ACLM” if you prefer not to make a credit card payment.
- Once the application has been submitted, it will be reviewed by three independent reviewers who will evaluate the responses and score them using ACLM’s Program Certification rubric. Questions are weighted differently depending on their importance. The final, total score will be the sum of the averages of the three reviewer’s scores for each question multiplied by the weight for each question. Applicants may be contacted by reviewers for follow-up information during the review period. Applicants will receive feedback on their applications.
